The presentation identifies the most frequent technical and operational causes, and steps crews and operators can take to address them. Many of the problems stem from a fundamental lack of awareness about the environmental loads for which anchoring equipment is designed, with operational use often exceeding the parameters on which class approvals are based.

One technical issue involves anchor winch brake bands. Many anchor losses are reported due to loss of windlass brake power and uncontrolled release of chain. Brakes are often not adjusted according to makers’ recommendations, which can accelerate corrosion of the drum and wear of the brake band lining, reducing brake capacity.

Håkon Skaret, principal senior engineer, DNV GL, said: “Every anchoring operation is unique. Crew and officers need to be aware of the risks and assess them thoroughly, especially in heavy weather. In almost half of the cases we saw that environmental risk factors, such as the weather, strength of the currents and water depth, played a significant role in the loss. To help shipboard personnel in this respect, shipping companies can make sure that procedures for safe anchoring operations are implemented in their safety management systems.”
